Register for a COVID-19 Vaccine from UPMC

UPMC is committed to delivering as many COVID-19 vaccines as soon as possible. We are encouraged by the community interest in receiving the vaccine, which is a critical step in controlling the pandemic and saving lives.

We are currently vaccinating those people who meet the Pennsylvania Department of Health’s Phase 1a criteria – which includes people over 65 and people with medical conditions that put them at greater risk of COVID-19 complications. If you are in Phase 1a and wish to receive a vaccination, we encourage you to register using the form below.

Vaccine supply from the state and federal government is still limited, but by registering now we will be able to contact you about scheduling an appointment when vaccine is available. Please be patient – we want to vaccinate as many people as possible and will do so quickly once we receive vaccine from the Commonwealth of Pennsylvania. There is no way to get vaccine other than the Commonwealth, and vaccinating the public will take weeks to months, not days.

Step 2: If you are eligible, please complete and submit the registration form below

If you complete Step 1 and are eligible for the vaccine at this time, please complete the registration form below. Each individual will need to submit their own form in order to be registered and contacted as vaccine becomes available.

Please Note: If you have limited access to online services or technology, or you need assistance in registering, you can call 844-UPMCVAC (844-876-2822) between 7 a.m. and 7 p.m., seven days a week. We expect high call volume on this line. To keep this option open to those who need it most, please use the online registration process if you can.

Step 3: Once you submit the form, you will receive additional communications from UPMC

Once you submit your form, you will receive a confirmation email to the address you provided, as well as ongoing updates on vaccine availability and distribution. As we help you prepare for your vaccine, you will also receive emails or text messages from UPMC Vaccine Scheduling and UPMC’s Certify Care system. You can also access answers to "frequently asked questions" about the COVID-19 vaccine and the vaccine scheduling process.

While waiting for a vaccine and after receiving the vaccine, please continue to wear a mask that covers your mouth and nose, practice physical distancing, and wash your hands frequently, as these measures prevent the spread of COVID-19 and save lives.
